IMPORTANT NEW INFORMATION FOR THIS SEASON!
All athletes participating on a Duval School District Athletic team must be covered by an insurance plan. Insurance company name and policy number is to be passed on to your child's coach. If you do not have coverage for your child, consider the student insurance plan offered in the beginning of the year packet that each student receives. Extra copies of these forms are available in the front office.
All athletes must have a current 2007-2008 sports physical forms

$3.00 Adults, $2.00 Students
Free for any child age 4 and under.
Free for any school coach with their FHSAA card
